Chronic Disease Education Program (CDEP)
The Chronic Disease Education Program is formerly known as the Diabetes and Heart Health Program, Prairie Health Matters and the Diabetes Education Resource.
What does the CDEP do?
This program will help you learn about healthy lifestyles and skills to help you manage your conditions. It offers support through individual appointments or group classes. A variety of topics are covered including:

Can I bring a support person?
Participants are welcome to bring a friend, family member or other support person to their appointment or any group classes.
Is there a cost?
The program is available at no cost.
What type of provider would I see and where would I see them?
Your appointment would be with a registered nurse and/or registered dietitian. The nurse and dietitian teams travel to various communities and First Nation communities throughout Prairie Mountain Health on a regular basis.
All services are confidential.
Are there group classes available?
Group classes are hands-on and can help you learn how to manage your condition. The classes have between 3 and 10 people which allow for time to talk with others who have similar questions and concerns.

You can call us yourself and make an appointment at 1-877-509-7852 (toll free).
Or you can talk to your doctor, nurse practitioner, or another healthcare provider and they can send us a referral. We will then contact you.
