Indigenous Health

Indigenous Health is developing relationships between Indigenous patients/residents/clients, communities and their health care providers to achieve health equity and culturally safe health care in the Prairie Mountain Health region.

Indigenous Health Patient Advocate provides/assists with:

• emotional support
• spiritual and cultural care
• advocacy
• access resources
• discharge planning
• care plans
• facilitates communication
• cultivate a relationship between Indigenous patients and their health care providers that is culturally sensitive and promotes culturally safe care and interactions

The Indigenous Health Patient Advocate can also:

Help improve continuity of care by raising awareness and promoting the following checkpoints prior to discharge:

• Does the patient require MEDICAL EQUIPMENT?
• Does the patient require HOME CARE SERVICES?
• Does the patient require DRESSING CHANGES?
• Does the patient have TRANSPORTATION HOME?
• Does the patient have FOLLOW-UP APPOINTMENTS booked?
