Health Promotion Community Development

Who we are: What is Health Promotion?

Working together to improve the health of individuals and communities one step at a time.

Health Promotion is about working with people and communities to create conditions where everyone has a fair chance to be as healthy as possible.

It means giving people the tools, information, and support they need to take care of their health and well-being.

It’s not just about telling people what to do — it’s about creating environments, communities, and policies that make the healthy choice the easier choice.

We all have a role to play in promoting health!

We help to improve the health of communities by:

  • Engaging community members by building trust and rapport
  • Improving knowledge and skills through free online and in-person courses, health fairs, information sessions, teen clinics etc…
  • Building partnerships with service groups and communities in Prairie Mountain Health and Provincially
  • Connecting to resources including health promotion programs and services
  • Creating and supporting policies to improve health
  • Providing support to assess a community’s health needs, and work together to support community action

What affects health?

Social Determinants of Health

  • Race & racism
  • Social supports & coping skills
  • Biology & genetics
  • Access to health services
  • Employment & working conditions
  • Education & literacy
  • Healthy Behaviours
  • Income & social status
  • Gender
  • Physical environments
  • Culture
  • Childhood experiences

The social determinants of health are the building blocks of health – they shape the conditions in which people are born, grow, live, work, and age.

These factors can make it easier – or harder – for someone to be healthy. For example, it’s easier to eat healthy if you can afford fresh food and live near a grocery store.

Addressing the social determinants of health is fundamental for improving the health of individuals and communities.

At every stage of life, our health is affected by the environment in which we live, work, and play and the choices that we make.
