Adult Day Program (ADP)
What does the Adult Day Program (ADP) Offer?
- A welcoming space where seniors can spend the day hanging out and meeting new people in a shared and fun setting.
- ADP is a community program open to Home Care clients that can provide respite to family and caregivers.
- Opportunity for older adults to meet other people and enjoy recreational activities away from home.
- Consists of planned activities designed to promote well-being through social, mental and recreational activity
- Supervised therapeutic activities (bingo, bible study, exercises and more)
- Offers a personalized approach
- Meals and snacks offered
- Help you to be able to stay at home longer
- Strengthen people’s ability to function within their own home by preventing physical and mental deterioration, which so often results from social isolation and loneliness.
Admission
The Home Care Case Coordinator will determine if you are eligible for the ADP and evaluates your needs each year (or more often if needed).
At times there may be a waitlist for the program. The waitlist is based on the date you applied and your priority level. The ADP Worker will contact you when a spot is open.
What we ask from you while enrolled
- Bring any equipment that you will need (e.g. wheelchair, walker, incontinence supplies).
- Bring any hygiene supplies and a change of clothes (please label).
- Bring medication if you need to take it during the program. Medications cannot be stored at the program and staff cannot help you take them.
- Please leave valuables and money at home. Prairie Mountain Health is not responsible for any personal items. You will be told if you should bring any money for an outing.
- You will be given a calendar of events each month. The events will vary throughout the month.
- Contact the ADP worker when you are unable to attend a session.
- Snacks and/or meals may be offered, depending on how long the program runs. Special dietary needs and individual preferences will be met when possible.
- In order to attend, clients who smoke need to be independent with their smoking needs (i.e. able to go to the smoking area on their own and safely return to the program).
- Make plans to get to the program and back home.
- If you are unable to make your own plans to get to and from the program, then please speak to the ADP Worker of Home Care Case Coordinator.
- Drop -off and pick-up times are to be within the program hours.
- You are to cancel your ride if you cannot go to ADP on your scheduled day (i.e. when you are sick).
- Fees are due each month. You will be given a bill to be paid at the Health Centre’s business office. Missing a payment will result in a review of services.
- If you want to change the day(s) you attend the program, talk to the ADP Worker. If there is spot open on your new requested day, you will be able to switch days. If there is no spot open, you will be placed on the waitlist.
- ADP will be cancelled if there is bad weather (e.g. severe wind chill, closed roads). You will not be charged the daily fee.
