My Health Team Evolves In Swan River Valley Area

Prairie Mountain Health (PMH) is pleased to announce that two additional primary health care staff have joined the shared care model in the Swan River area, which is part of the provincial initiative known as ‘My Health Team’.

A primary care pharmacist as well as a licenced Clinical Assistant, based out of the Swan Valley Primary Care Centre, are now working with My Health Team—Swan Valley and Area.

Brittany Schroeder is supporting medication management for clients with chronic and complex health conditions. The primary care pharmacist works together with other health-care providers to ensure that each patient is on the best possible medication plan.

A licenced Clinical Assistant, Adeydayo Ojo, is the most recent addition to the Team. Clinical Assistants (CA) are health care professionals, educated within a medical model, who practice under the supervision of a licensed physician. This will be the first time a CA will be practicing in the Swan River area.

These new positions, all funded through PMH, help strengthen My Health Team —Swan Valley and Area. They work together with a team of health-care providers that already provide services through the initiative such as a doctor, nurse practitioner, community health nurse, shared care community mental health worker and primary care community outreach worker.

Based in Swan River, My Health Team—Swan Valley and Area is a partnership of the fee-for-service physician group based at the Swan Valley Primary Care Centre (SVPCC), along with PMH region and Sapotaweyak Cree Nation Health Authority. As part of the initiative, physician/ health care providers also visit weekly to the Sapotaweyak Health office.  

Through this collaborative approach, health-care professionals provide coordinated care in a more timely manner for patients in the northern part of the region. 

“Team members support and assist you to access accurate health care information to help you make informed choices about self-care across the lifespan. It places a renewed focus on prevention and your overall wellness, not just treatment and illness,” stated Dr. Rafiq Andani, on behalf of the physician group at SVPCC.

The Swan Valley and Area My Health Team is one of two My Health Teams within PMH currently providing service through the shared care model. My Health Team Brandon, a partnership with the Western Medical Clinic and Meridith Medical Clinic, commenced in late 2014.
